Emergency Service Disclaimer
is a digital medical identification service. We are not a medical provider, an emergency response agency, or an insurance company. Our service provides information to third parties (such as first responders) at your direction. In any medical emergency, always contact emergency services (911) immediately.
1. Acceptance of Terms
By creating an account, selecting a membership plan, or using the
QR code system, you agree to comply with and be bound by these Terms of Service. If you do not agree, you may not use the service.
2. Membership & Subscriptions
We offer one free tier and two paid membership plans, each billed monthly. By subscribing, you agree to the specific limits and features of your chosen plan:

Up to 5 Profiles
- The Free plan includes one profile with core medical sections at no cost. Paid plans (Silver and Gold) unlock all profile sections, PDF export, selective data sharing, scan-log history and automatic SMS scan alerts.
- Profiles may represent family members or pets. Silver supports up to 2 profiles; Gold supports up to 5 profiles with a family dashboard and independent profiles per member.
- Paid plans are billed monthly. We do not currently offer annual billing.
- Subscriptions automatically renew each month unless cancelled via your account dashboard or the mobile app.
- You may cancel at any time; paid access remains active until the end of the current billing month.
- Plan changes take effect at the start of your next billing cycle. Downgrading may reduce the number of available profiles and features.
3. Accuracy of Medical Information
You are solely responsible for the accuracy, completeness, and timeliness of the information entered into your profile. You agree to review and update your medical data regularly.

4. Physical Products & Delivery
Membership plans include the ability to order physical wallet cards and windshield decals.
- Products are personalized based on the data in your selected profile at the time of order.
- Once a physical order is processed, modifications cannot be made.
- Shipping times vary by location; we are not responsible for carrier delays.
5. Privacy & Security
Your use of the service is also governed by our Privacy Policy. We do not track your location or send automatic alerts. Information is only shared when a responder scans your QR code or accesses your secure medical PDF link.
6. Limitation of Liability
To the maximum extent permitted by law,

7. Messaging Terms & Conditions
These Messaging Terms govern the SMS notifications sent by
. Our messaging program sends transactional emergency alerts only — a text message is sent to a designated emergency contact when a user's medical ID QR code is scanned or their secure link is opened. We do not send marketing or promotional messages.
General & Opt-In
By opting in to the service, you agree to receive automated transactional text messages from
at the mobile number you provide. When you opt in, we may send a message to confirm your sign-up. Your consent to receive messages is not a condition of any purchase.
Message Frequency & Rates
Message frequency varies and depends solely on how often the associated medical ID is scanned. Message and data rates may apply. If you have questions about your text or data plan, contact your mobile provider. Your mobile provider is not liable for delayed or undelivered messages. We reserve the right to change the phone number or sender from which messages are sent.
Cancellation (Opt-Out)
You may unsubscribe at any time by replying STOP to any message. After you unsubscribe, we will send a message to confirm that you have been unsubscribed, and no further messages will be sent. To resume messages, opt in again as you did the first time. You may also email support@medtagid.com to request removal.
Help
For help, reply HELP to any message or email us at support@medtagid.com and we will respond with assistance and instructions on how to unsubscribe.
Transfer of Number
Before changing your mobile number or transferring it to another person, you agree to either reply STOP from the original number or notify us of your old number at support@medtagid.com. Informing us of these events is a condition of using this service.
Privacy
Phone numbers collected for SMS notifications are never sold, shared, or used for any purpose other than sending emergency scan alerts. For questions about your data or our practices, see our Privacy Policy.
Changes to These Messaging Terms
We reserve the right to change or terminate our messaging program at any time, and to update these Messaging Terms at any time. Such changes are effective immediately upon posting. If you do not agree to a change, you should cancel your enrollment by replying STOP. Your continued enrollment following any change constitutes acceptance of that change.
